To initiate the Advancement documents through DocuSign PowerForms, please complete the survey below.

Grad Division Filing Deadlines: https://grad.uci.edu/current-students/filing-deadlines/ 

After you complete the survey:
  • Expect an email from dse_NA3@docusign.net within 1-2 business days. This will be your official Advancement documents that will be routed for signatures.
  • When the document has been reviewed and signed by all parties, it will automatically be sent to Grad Division for review.
  • Approved Advancement documents will be sent to the Grad Program Coordinator and you will be notified of the status.
