Dear Colleagues - Instructors of record are invited to complete this brief feedback form to allow the campus to better understand your educational perspectives and needs, including thoughts on student success. We will be releasing additional forms tailored to teaching assistants and other members of the instructional team. Completion is anonymous, and we are not collecting information on who is/is not accessing this form.

It is possible that the responses will be published/presented in some fashion. In this case, all responses will be aggregated where possible and will include no identifiable information.
